IN-SPIRO REGULATIONS

  1. Clients who decide to undergo therapy at IN-SPIRO must accept the practice’s regulations and the GDPR.
  2. Clients schedule appointments by phone at 603 650 095 or via the in-spiro.pl website. When booking online, the client must provide an email address. A notification of the scheduled appointment, a reminder, and a link to cancel the appointment will be sent to the email address provided.

  3. IN-SPIRO is not responsible for items left in the waiting room.

  4. Clients are required to accurately complete the Card, which serves as their medical records. If they have any doubts about their health, they should consult their doctor before undergoing the treatment.

  5. Children under 18 or incapacitated individuals may receive treatments in the presence of a guardian, unless their guardian consents to the treatment without their presence.

  6. Clients for their first treatment are asked to arrive at least 5 minutes before their treatment, as this is the time needed to complete the Card. The client is required to arrive on time for their scheduled appointment.

  7. In the event of a delay, the treatment time may be shortened by the delay.

  8. In the event of a delay due to the therapist’s fault, the treatment will be extended by the delay time or the client will receive financial compensation in the form of payment of the treatment fee reduced by 10% of the treatment cost.

  9. The treatment lasts from 50 to 60 minutes. The client prepares for the treatment by wearing comfortable, non-restrictive underwear. Structural Integration Therapy requires access to the skin, and the therapist does not work through clothing.

  10. The client is asked to mute or turn off their phone during the treatment.

  11. The client should immediately inform the therapist if they feel unwell or anxious during the treatment.

  12. IN – SPIRO does not sell treatment packages. Payment for the visit is due after each completed treatment. IN – SPIRO reserves the right to refuse therapy to clients with active infections (colds, flu, tonsillitis, etc.), to individuals who are intoxicated or under the influence of drugs, or to individuals who are unjustifiably disruptive.

MEDICAL DOCUMENTATION

  1. In connection with the provision of healthcare services, IN – SPIRO maintains medical records. These records will be prepared in Polish.
  2. The procedure, conditions for maintaining medical records, and the manner of sharing them with third parties are specified in applicable regulations.
  3. Extracts, descriptions, or copies of medical records will be prepared and released upon receipt at the requestor’s expense.

CUSTOMER CONSENT

  1. The client consents to the processing of their personal data by IN – SPIRO, Emilia Hrehoruk, Stare Faszczyce 50K, 05-870 Błonie, for the purposes necessary to provide physiotherapy services.
  2. Providing personal data is voluntary. If the client does not provide their personal data, the office has the right to refuse to perform the procedure. The basis for data processing is consent.
  3. The client has the right to withdraw consent at any time. Personal data will be processed for the purpose of possible withdrawal of consent, and after such withdrawal, for the limitation period for claims against the data controller, in accordance with applicable law. The client has the right to request access to their personal data from the controller, its rectification, erasure, or restriction of processing, as well as the right to lodge a complaint with a supervisory authority.
